Annual Self Review

Before you schedule your annual performance review, we'd like you to rate yourself for 10 different metrics. You can find descriptions of each below.


There are 5 rating levels for each metric - Rockstar (5), Excellent (4), Above Average (3), Average (2), and Poor (1). After you've completed your review, a self performance score will be calculated. This will be taken into account when your manager reviews your performance.

Below is a Description of Each Metric to Score Yourself On.

Quality: Measured by the staff members ability to complete work to operational standards without requiring extensive quality assurance reviews and corrections.

Awareness: Measured by the staff members ability to track task workload, complete tasks by expected dates, report on bottlenecks and preemptively identity issues.

Knowledge: Measured by the staff members ability to ask questions, continuous learning initiatives, implementation of intellectual property and demonstration of technical knowledge within their department.

Problem Solving: Measured by the staff members ability to introduce solutions before problems and explore multiple options.

Ownership: Measured by the staffs ability to take ownership of their work by ensuring follow-through on projects to obtaining a results driven goal.

Teamwork: Measured by the staff members ability to communicate and work with the team by demonstrating unity and helping each other thrive in their individual roles.

Simplicity: Measured by the staff members ability to implement sustainability within their role by identifying inefficient process with understanding that speed is process driven.

Integrity: Measured by the staff members ability to be admirable, “do the right thing”, candid, and speak their mind.

Innovation: Measured by the staff members ability to find a better way to do their job by challenging assumptions to foster innovation.
